Interventional radiology (IR) is one of the more hands-on paths in imaging, and it’s easy to see why techs want to make the move: more procedural variety, closer work with physicians, and a more active role in patient care than general radiography allows. The harder part is figuring out how to actually get there and what employers expect once you apply.
This article covers the certification path, the skills that matter most, and what working in IR really looks like day to day.
How It Differs from General Radiography
Since interventional radiology techs support live procedures rather than capturing standalone diagnostic images, moving over from general radiography takes more than switching departments. Departments look for candidates who can operate advanced fluoroscopy and angiography equipment, handle unpredictable, fast-moving cases, and work under more clinical pressure than a typical diagnostic shift. That is the gap the certification and experience steps below are built to close.
The Path to Becoming an IR Tech
The path to being an interventional radiology technologist is straightforward, but it does take time and intentional steps. Here is what the typical journey looks like.
Complete an Accredited Radiography Program
Start with a radiography program accredited by the Joint Review Committee on Education in Radiologic Technology (JRCERT).¹ Most programs award an associate degree over about two years, though length varies by school, and cover imaging fundamentals, anatomy, radiation safety, and patient care. Radiologic technologists typically need this degree before they can pursue certification.²
Pass the ARRT Radiography Exam
After graduating, you will need to pass the American Registry of Radiologic Technologists (ARRT) Radiography exam to earn your Radiography (R) credential. This is the standard certification for radiologic technologists and the baseline requirement for entering IR. Some states also require a separate license, so check the rules in your state.
Build Experience in Imaging
Prior imaging experience is strongly preferred in most IR departments. Time spent in diagnostic X-ray, surgery imaging, computed tomography (CT), or a cath lab will strengthen your application. Fluoroscopy exposure in particular is highly valued.
That said, many hospitals, especially community hospitals, will hire motivated newer grads into IR trainee positions when staffing demand is high. If you are earlier in your career, it is still worth applying.
Choose Your Path into IR
From here, there are two ways in. The first is on-the-job training. Many hospitals cross-train experienced rad techs directly into IR, especially when demand is high.
The second is the Vascular-Interventional (VI) credential through the ARRT. Earning it means already holding your Radiography (R) credential, then documenting at least 200 vascular interventional procedures and passing a specialty exam.³ It signals formal IR training and can open doors at larger or more specialized facilities.
Many techs enter through on-the-job training first and pursue the VI credential later.
Skills IR Tech Employers Look For
Getting into IR requires more than the right credentials. Employers screen for a specific set of skills before they hire. Here are the ones that matter most.
Technical Skills
- Sterile Technique. IR procedures involve direct access to blood vessels. Keeping the procedure area clean and sterile is a basic patient safety requirement.
- Fluoroscopy and C-Arm Operation. IR runs on live X-ray imaging. You need to be comfortable operating imaging equipment and adjusting it during active procedures.
- Radiation Safety. IR techs are exposed to more radiation than most imaging roles. You need to know how to reduce dose and protect yourself and the patient.
Clinical Skills
- Patient Monitoring. Many IR patients are critically ill. You will watch their vitals throughout the procedure and respond fast if something changes.
- Hemostasis Awareness. After vascular procedures, you need to understand how to manage bleeding at the access site and what to watch for afterward.
- Adaptability. Procedures can shift from routine to emergency quickly. Employers want techs who stay focused when that happens.
Soft Skills
- Clear Communication. You work with physicians and nurses throughout every case. Being clear and quick with communication keeps things running safely.
- Composure Under Pressure. Things do not always go as planned in IR. Staying calm in high-pressure situations is one of the most valued traits in this role.
- Attention to Detail. Small errors in documentation or image labeling can have real consequences. Accuracy matters in every part of the job.
Where to Start: Hospital or Outpatient IR
For candidates without prior IR experience, hospital departments are usually the more realistic entry point. They are more likely to cross-train new rad techs on the job, pairing them with experienced staff while they build procedure hours toward a VI credential later. Outpatient centers tend to hire techs who already have IR experience, since smaller teams and steadier schedules leave less room for on-the-job ramp-up.
That does not rule out outpatient IR early on. It just means the more common sequence is hospital IR first, VI credential second, and outpatient IR once you have the procedure hours and confidence to back it up.
