Under general supervision, responsible for the total nursing needs of residents. The Charge Nurse ensures a safe and healthy environment for residents, staff, and visitors to participate in the residents daily life rhythm. Develops assignments in coordination with the care partners and keeps each team member accountable for resident care and satisfaction. Duties and assignments may be adjusted at the discretion of the Director of Nursing Services and/or designee.

Essential Functions

  • Facilitates nursing care provided to all residents and includes residents in decision making processes;
  • Actively engages residents in participation in daily activities which are part of resident?s rhythm of daily life (including desired hygiene, grooming, and mobility);
  • Assesses resident needs and promotes the highest functional level of independence possible (keeping in compliance with state and federal regulations, and the community?s safety procedures);
  • Supervises and mentors care partners by providing support, encouragement, and counseling for growth as appropriate;
  • Makes frequent rounds and utilizes the nursing process to identify resident conditions that require further evaluation and treatment? by a physician (reports relevant and significant changes the MD);
  • Responds to emergency situations, carries out physician orders, and documents the event, interventions, and outcomes;
  • Safely administers medications and treatments to residents per physician?s orders and documents as appropriate;
  • Follows-up on resident medication needs, specifically advocating resident safety;
  • Reports and documents vital information on the 24-hour report;
  • Actively engages and participates in the community?s QAPI program;
  • Completes risk management, progress notes, assessments, weekly summaries, and care plans;
  • Coordinates the resident?s plan of care with the interdisciplinary team (IDT), physicians, medical personnel,? and resident?s family/responsible party;
  • Interacts with other departments such as laboratory, radiology, dietary, facilities, information technology, pharmacy, social services, activities, etc;
  • Collects lab specimen for testing, obtains and assesses vital signs, weights, I/O?s, etc;
  • Makes necessary documentation corrections as a result of medical record audits;
  • Proactive in the move-in and move-out procedures for each resident;
  • Will build genuine relationships with residents through discussion and review of their history;
  • Checks on operating condition of equipment and notifies appropriate personnel when necessary;
  • Provides effective and courteous service to all residents, family members, guests, and team members; is clear in communication and handles conflicts appropriately;
  • Attends and actively participates in in-service training and meetings as scheduled; incorporates training received into daily work;
  • Participates in move-in and move-out procedures;
  • Maintains daily records consistent with regulations;
  • Promotes and protects the rights of each resident;
  • Performs other related essential duties as required;

Minimum Requirements

  • Education
    • Completion of Licensed Nursing Program (RN)
  • Experience/Training 
    • Clinical observation and assessment skills
    • Proficient computer skills necessary to complete documentation, review electronic health records and communicate with team members.
